TORCH Test in Pregnancy: Screening for Toxoplasmosis, Rubella, CMV & Herpes

During pregnancy, your body undergoes incredible physiological adaptations to protect your growing baby. But certain common viruses and parasites that cause barely a sniffle in adults can cross the placenta and cause severe complications for a developing fetus. To screen for these hidden microbial threats, doctors order the TORCH Panel.

What Does TORCH Stand For?

TORCH is an acronym for a cluster of infectious agents known to cause congenital complications if contracted during pregnancy:

  • T – Toxoplasmosis: A parasite commonly spread through contact with cat feces, contaminated garden soil, or undercooked meats.
  • O – Others: Includes Syphilis, Hepatitis B, Varicella (chickenpox), and Parvovirus B19.
  • R – Rubella: Also known as German Measles. Can cause congenital cataracts, deafness, and heart defects if contracted in the first trimester.
  • C – Cytomegalovirus (CMV): A widespread herpes-family virus that is the leading viral cause of congenital hearing loss.
  • H – Herpes Simplex Virus (HSV-1 and HSV-2): Can cause severe neonatal infection if active lesions are present during delivery.

Decoding IgG vs. IgM: What Do Your Antibodies Mean?

When you receive your TORCH report, each pathogen will have two scores: IgG and IgM. This often causes unnecessary panic among mothers:

Antibody PatternWhat It MeansClinical Status
IgG Positive, IgM NegativePast Immunity (Safe!)You had the infection or vaccine years ago. Your body has protective antibodies. Excellent news!
IgG Negative, IgM NegativeNo Immunity (Susceptible)You have never been exposed. You are safe now, but need to avoid exposure (e.g. get vaccinated for Rubella before pregnancy).
IgM Positive (With or without IgG)Recent / Active InfectionIndicates a recent exposure. Your gynaecologist will order confirmatory avidity testing.
