Polycystic Ovary Syndrome (PCOS) is one of the most common endocrine disorders among young Indian women, affecting roughly 1 in every 5 women of reproductive age. Yet countless women struggle in frustration: stubborn chin hair (hirsutism), painful cystic jawline acne, unexplainable belly fat, and menstrual cycles that arrive once every 45 to 60 days.
While an ultrasound of the pelvis can look for a "string of pearls" appearance of immature cysts, ultrasound alone cannot diagnose PCOS. The true biological driver lives in your pituitary hormones: specifically, the balance between Luteinizing Hormone (LH) and Follicle Stimulating Hormone (FSH).
The Normal Dance: LH and FSH in Harmony
In a healthy menstrual cycle:
- FSH (Follicle Stimulating Hormone) rises first. It tells your ovaries to mature a few follicles and pick the best egg.
- LH (Luteinizing Hormone) waits quietly in the background until mid-cycle (Day 14), when it surges rapidly to trigger the release of the mature egg (ovulation).
In a healthy woman tested on Day 2 or Day 3 of her period, the ratio of LH to FSH is approximately 1:1 (for example, LH is 5 mIU/mL and FSH is 5 mIU/mL).
What Happens in PCOS? The 2:1 or 3:1 Inversion
In women with PCOS, the pituitary gland continuously pumps out abnormally high levels of LH right from the start of the cycle. The ratio flips to 2:1 or even 3:1 (e.g., LH is 15 mIU/mL while FSH is only 5 mIU/mL)!
This high LH prevents the ovary from ever selecting a single dominant egg. Instead, dozens of small follicles stall halfway, forming fluid-filled sacs (cysts). Worse yet, excess LH stimulates the ovarian theca cells to overproduce male androgens (Testosterone), causing acne and unwanted hair growth.
| Parameter (Tested Day 2 or 3 of Period) | Normal Cycle | PCOS Pattern |
|---|---|---|
| LH to FSH Ratio | ~ 1:1 | 2:1 to 3:1 or higher |
| Total Testosterone | 15 – 70 ng/dL | Elevated (> 70 ng/dL) |
| AMH Level | 1.5 – 3.5 ng/mL | High (> 4.0 – 10+ ng/mL) |
