Elevated Alkaline Phosphatase (ALP): Is the Issue in Your Liver or Your Bones?

You receive your routine Liver Function Test results, and most numbers look reasonable—except for one line item with a bold red flag: Alkaline Phosphatase (ALP): 280 U/L (Normal is typically 44 to 147 U/L). Your search engine tells you it could mean a liver tumor, a blocked gallbladder, or a bone disease. Panic ensues.

Before jumping to worst-case conclusions, understand that Alkaline Phosphatase is one of the most tissue-diverse enzymes in your body. It exists in large amounts in two primary locations: the bile ducts of your liver and the osteoblast cells of your bones.

The Detective Test: Gamma-Glutamyl Transferase (GGT)

How does a doctor know whether your elevated ALP is coming from your liver or your skeleton? They run a companion test called GGT (Gamma-Glutamyl Transferase):

  • If BOTH ALP and GGT are High: The source is definitively the LIVER and BILE DUCTS (e.g., gallstones obstructing bile flow, medication toxicity, or biliary cirrhosis).
  • If ALP is High but GGT is Normal: The liver is completely innocent! The excess enzyme is coming from BONE TURNOVER.

When High ALP is Completely Normal and Harmless!

There are two common life phases where soaring ALP levels are not only harmless, but an indicator of healthy biology:

  1. Growing Children & Teenagers: During adolescent growth spurts, bones are remodeling and expanding rapidly. A healthy 13-year-old can have an ALP reading three times higher than an adult!
  2. Third Trimester of Pregnancy: The human placenta manufactures its own unique isoenzyme of Alkaline Phosphatase. During late pregnancy, maternal ALP routinely doubles.
